Can Somebody Help Meeeeeee?

Years ago I sang a mdrigal by William Hawley with text by Torquato To. I remember most of the text, but I'm not 100% sure... I've done the most ezhaustive net search I can come up with, and still haven't found it. Here's what I remember:

Vita de la mia vita
Tu mi somigli pallidetta oliva
O rosa scolorita
Ne di belta sei prova
Ma in ogni aspetto tu mi sei gradita
O lusinghiera o schiva
E se mi segui o fuggi
Soavemente mi consumi e struggi

I'm not completely sure of the spelling, either. Is anyone familiar with this piece?
