This is an English or Shakespearian sonnet. Its form is three quatrains and a couplet. In the first two quatrains the speaker indicates that he is not concerned that his relationship is ending. In fact he is happy to have gotten off so cleanly. In the next quatrain he uses personification to show that his earlier words were a result of pride and not a reflection of his feelings. Love is represented as a person on his deathbed. In the last couplet the speaker denies the first two quatrains by showing that he really wants his relationship to survive. He says she can still bring love back to life.
Good luck
